New Patient Form 

To help make your visit go smoothly, we ask that you complete our patient intake form before your appointment. 

A few tips: 

  • One form per patient. 
  • Bring your completed form, photo ID, and insurance card with you. 
  • If you’d prefer to fill out your form in-office, please arrive 10–15 minutes early.

Insurance Plans We Accept 

North Coast Eye Care participates in most major vision and medical insurance plans. Because coverage can vary widely between plans, we recommend giving us a call before your appointment so we can confirm your benefits.
